REGISTRATION INFORMATION

Forms are requested by August 14, 2008.
Players will not be allowed on the ice until all paperwork has been completed.
The printable forms required for registration are listed on the left and below. Each form will open in a new window. If you would prefer to have the forms mailed to you, phone (425) 641-3265.

3. WWFHA Consent to Travel
Parent/guardian must sign and date this form.

4. Code of Conduct & Publicity Waiver Contract
Player and parent/guardian must sign and date the Code of Conduct. The Publicity Waiver Contract is optional.

5. USA Hockey Consent to Treat & Medical History Form
Player and parent/guardian must sign and date the Consent to Treat form. The medical history form is optional.

7. Birth certificate
Not necessary if we have one on file from a previous season.

8. Copy of I94 or green card if not a US citizen Not necessary if we have one on file from a previous season.

Online USA Hockey registration

All WWFHA players, coaches and team managers are required to register online with USA Hockey.

[Note: if you are playing for another association as well as WWFHA, and that association doesn't require you to register online, then don't register online or you'll end up paying twice. Fill in the player signup form and note which other association you'll be joining, and we'll take care of it.]

Complete the online registration process, including credit card payment of the $30 USA Hockey membership fee. You'll receive a confirmation page that confirms you are registered with USA Hockey and your Affiliate.

Mail your confirmation page, or email it to the WWFHA Registrar, to be claimed by the program and placed on a team. This step is required to ensure that all USA Hockey benefits are in place.

You must be 18 years or older to participate in Online Registration. If you are not 18 years of age or older, a parent or guardian must complete this process on your behalf.

2008-2009 Program Fees TBA

Checks for program fees should be made out to WWFHA. Pay your fee in a lump sum and get a $50 discount.
If you would like to arrange a different payment schedule, contact the treasurer.
