Registration Information


Forms are requested by September 1, 2012 for the 2012-2013 season. Players will not be allowed on the ice until all paperwork has been completed. The printable forms required for registration are listed below. Each form will open in a new window. If you would prefer to have the forms mailed to you, phone (206) 855-5636. 4. Birth certificate (not necessary if we have one on file from a previous season)
5. Copy of I94 or green card if not a US citizen (not necessary if we have one on file from a previous season).

* Online USA Hockey registration
All WWFHA players, coaches and team managers are required to register online with USA Hockey.
Complete the online registration process, including credit card payment of the $39 USA Hockey/PNAHA membership fee.
You'll receive a confirmation page that confirms you are registered with USA Hockey and your Affiliate (WWFHA).
Mail your confirmation page to WWFHA, P.O. Box 77687, Seattle, WA 98177, or email it to the WWFHA Registrar, to be claimed by the program and placed on a WWFHA team.
This step is required to ensure that all USA Hockey benefits are in place.

You must be 18 years or older to participate in Online Registration. If you are not 18 years of age or older, a parent or guardian must complete this process on your behalf.

Goalies fees are $450 for Wild 1 or 2 and $250 for the Phoenix.

Checks for program fees should be made out to WWFHA, and mailed along with your completed Payment Schedule Agreement form to
WWFHA, P.O. Box 77687, Seattle, WA 98177.

Additional Costs

• All players will be required to contribute to a team fund to pay for their coaches' expenses. Our coaches are not paid to coach but are reimbursed for traveling expenses including travel to practices and games.
• Tournament costs are not included in the program fee. Each player will be charged for her share of the tournament entry fee plus her share of the team expenses. Teams pay for coaches' hotel rooms and an allowance of $100 per coach per night.
• Each player will usually have to pay between $80 and $120 to the team fund for out-of-town tournaments.
• There will also be personal expenses such as hotels and meals during tournaments. Washington WILD team expenses will range from about $50 to $80 per month to cover the cost of coaches' expenses, bus hire and tournament fees.
• If you would like to arrange a different payment schedule, contact the treasurer.
• Fees may be paid monthly by arrangement with the treasurer.
• We are actively seeking sponsorships to reduce fees.
• Players joining after the start of the season pay a pro-rated program fee plus a $35 late registration fee.
